Preoperative assessment of lower extremity varicose veins using non-contrast 3DCT-venography

BACKGROUND: The objective of the present study was to determine the efficacy of non-contrast three-dimensional computed tomography (3DCT) in the preoperative evaluation of varicose veins in the lower limbs.
METHODS: In total, 166 lower extremities of 134 patients with varicose veins who underwent duplex ultrasonography (US) and non-contrast 3DCT preoperatively at our department were included. Non-contrast 3DCT was performed to obtain foot apex image in the supine position, whereas US was performed in the standing position; laboratory findings obtained with both testing methods were compared. Superficial vein description was evaluated using non-contrast 3DCT images, with image clarity classified into four levels: Excellent, Good, Fair, and Poor.
RESULTS: Superficial vein diameter and running/branching form were almost the same with both testing methods. The average diameter of the great saphenous vein targeted for treatment (N.=123) was significantly different on CT (7.3 mm) and on US (6.5 mm) (P<0.05). Clear images were obtained for 129 (77.7%) limbs, but partially unclear images were obtained for 26 (15.7%) limbs; the superficial veins of 11 (6.6%) limbs were difficult to evaluate.
CONCLUSIONS: Non-contrast 3DCT is useful for the preoperative evaluation of varicose veins in the lower limbs.
