Deep vein valves incompetence and treatment implications

Chronic venous disease due to deep vein valve incompetence is one of the most severe forms of chronic venous incompetence, leading to persistent ambulatory venous hypertension. The patient experiences pain, edema, varicose eczema, lipodermatosclerosis and eventually, in some cases, limb ulceration. If obstruction is ruled out, the most persistent cases of chronic venous disease often have deep vein valve incompetence as the underlying pathophysiology. A significant proportion of venous ulcers (8%) can remain unhealed after 5 years or more, and this disease also has major socio-economic consequences. This review discusses the pathophysiology and clinical management of chronic venous disease.
