The role of minimally invasive approach for open surgical thoracoabdominal aortic replacement

Less-invasiveness of endovascular thoracoabdominal aortic (TAA) aneurysm repair led to increased interest although cumulative experience remained short-termed. Alternatively, open surgical TAA replacement improved tremendously during the last years towards fewer exposure traumas and can be performed with excellent results at the present. However, an irrational persistence of an unexplained fear towards the open surgical repair of TAA aneurysms continues to exist. The review discusses the evolution of both open surgical and endovascular techniques towards less-invasiveness and evaluates the relevance of smaller incision and less surgical exposure in the context of safe and durable aortic reconstruction.
