Rhabdomyosarcoma is a malignant tumor arising from embryonic mesenchymal precursor cells of the striated muscle cells. Genitourinary tract rhabdomyosarcoma is a rare disease nearly exclusive to the pediatric population. Despite being a highly aggressive malignant tumor with rapid local growth, recent innovations in the chemotherapy field have greatly improved the prognosis of young patients. Diagnostic imaging plays an essential role in the management of these patients, both in the diagnostic phase and in the post-therapeutic phase, to evaluate any residual disease. We describe a case of embryonal rhabdomyosarcoma of the prostate in a 3-year-old boy treated at Santobono-Pausilipon Children’s Hospital of Naples.
