Anisakis un patogeno interdisciplinare

The ingestion of Anisakis larva can be a cause of infestation by the nematode, causing abdominal symptoms, or allergic sensitization to it. Anisakis allergy diagnosis is based on the skin prick test and specific IgE determination, but false positivity cases are frequent. This problem occurs in patients affected by chronic spontaneous urticaria or can be caused by cross reactivity with numerous allergens. The allergic component resolved diagnosis can be useful, but only Ani s1 genuine allergen is available in multiplex platforms. Ani s3 cross-reactive allergen is also detectable in microarray, but analogous tropomyosin of shrimp is available in single-plex platforms. Basophil Activation test has demonstrated a high diagnostic accuracy. A comprehensive diagnostic algorithm including this test enlarged to IgE versus another nematode as the Ascaris, to evaluate a possible primary sensitization to other parasites, can be a valid support to the diagnosis of Anisakis allergy.
