Superior labial frenectomy: a report of two cases

The labial frenum is defined as an anatomical structure that joins the lips to the alveolar processes of the maxillary or mandibular bone. Ideally, the insertion of the frenum should be located at the level of the mucogingival junction, so as not to interfere with the adhesion of the attached gingiva. A low frenal insertion may jeopardize the gingival health and complicate periodontal disease and hinder successful treatment (periodontal, orthodontic or prosthetic), either due to interference in the plaque control or due to a muscle pull. This type of situation can be solved by a simple surgical intervention called frenectomy which consists in performing a complete removal of the frenum, including its attachment to the underlying bone. The aim of this article was to present from two clinical cases the operative protocol of the upper labial frenectomy using the conventional technique. According to our observation, we can conclude that frenectomy of upper labial frenum using conventional technique represents a reassuring alternative to treat aberrant frenum. However, this technique has some limitations including: bleeding, discomfort for the practitioner, and duration of the procedure.
