Myxofibrosarcoma of temporal muscle, description and treatment

Myxofibrosarcoma in adulthood is the most common sarcoma in the extremities and torso. However, it is highly uncommon in the head and neck with little metastatic potential except those of high grade. Its initial study should be done by MRI scan and computed tomography (CT), and it should be confirmed by histopathological study. In this paper we present a case of new location of myxofibrosarcoma in temporal muscle thickness, treated by lesion extensive resection and reconstructed with the anterolateral microvascular thigh flap. This method provides us, free surgical margins and, without complementary treatment after decision of the Tumor Committee, the patient is free of disease after one year. When we face a high degree of malignancy myxofibrosarcoma, an aggressive initial surgical attitude together with a complementary radiotherapy treatment has been proposed as an absolute recommendation. However, the study and individualization of each patient makes us adopt different attitudes assessing the pathological anatomy of the tumor, the oncological margins and the patient’s basal health status.
