The mandibular canine commonly presents as a single rooted tooth though anatomical variations have been reported in some instances. A commonly occurring variation is a two rooted mandibular canine with two root canals. The knowledge of the anatomy of the root canal system is important in endodontic management of a tooth whenever necessary. The patient in the following case report presented to the dental clinic complaining of a missing lower tooth. Clinical examination revealed a retaining mobile lower left deciduous canine; however, the permanent canine was absent. This case report presents the endodontic treatment of an impacted canine with two roots and two root canals. The tooth was re implanted in its anatomical position and endodontic treatment has been done using the protaper system. Radiographic analysis was done preoperatively, intraoperatively and postoperatively. This case enlists the importance of ruling out anatomical anomalies pertaining to root canals prior to dental treatment.
