Platelet-rich fibrin: a novel healing adjunct for the oral and maxillofacial surgeon

Platelets play an essential role in hemostasis and tissue regeneration. Supra physiological doses of platelets were adopted in the 1990’s called platelet-rich plasma. Due to drawbacks associated with platelet-rich plasma, a second generation called platelet-rich fibrin was developed. This is a case series of arthrocentesis, extractions, enucleation of cyst, buccal mucosa harvest, and oral antral communication in which platelet-rich fibrin was used to augment the healing process. Some authors found that platelet-rich fibrin accelerates the healing of soft and bone tissue and reduces postoperative pain after extractions. Platelet-rich fibrin is also reported to cause less infection with its use. Lastly, platelet-rich fibrin was injected into the TMJ and a reduction in pain and increase in mouth opening was observed. Platelet-rich fibrin has been demonstrated to exert an anti-inflammatory environment. It has been shown to permit a rapid angiogenesis and is resistant connective tissue that is easy to work with. In conclusion, platelet-rich fibrin can stimulate the recruitment and proliferation of endothelial cells, gingival fibroblast, chondrocytes, and osteoblasts.
