Clozapine-induced acute interstitial nephritis

Acute interstitial nephritis can result from drug’s hypersensitivity reaction. This paper describes the development of an acute interstitial nephritis in a 25 years-old woman treated for the last 7 weeks with clozapine for a psychiatric disorder. Fever, elevated inflammatory markers and acute kidney injury were identified, and renal biopsy confirmed the diagnosis of interstitial nephritis. Patient experienced complete recovery after clozapine withheld. Clozapine is a highly effective antipsychotic drug, but can it causes fatal agranulocytosis and its hematological toxicity is well documented. We report clozapine-induced acute interstitial nephritis as another serious side-effect, suggesting that renal function monitoring should be considered in patient under clozapine treatment.
