A 57-year-old with a history of malignant hyperthermia (MH) was admitted following one-month history of unstable angina and found to have inferior wall ST-elevation myocardial infarction. Coronary angiography showed severe multi-vessel coronary artery disease with the culprit being a mid-right coronary lesion. Following preoperative multidisciplinary planning, the patient underwent three-vessel coronary artery bypass grafting. Her postoperative course was uneventful, and outpatient management was routine. Judicious periprocedural management of patients with a history of MH is important for successful cardiac surgical outcomes.
