Stercoral ulcerations are usually the result of pressure necrosis within the colorectum and often present with symptoms including severe constipation, hematochezia and abdominal pain. Endoscopic evaluation involves collecting tissue biopsies to evaluate for possible underlying malignancy. We present the case of a woman with stercoral ulceration and unusual symptoms of fecal incontinence and hematochezia for which endoscopic biopsies revealed a primary signet ring cell carcinoma of the rectum.
