Early vascular aging: prevalence in Russian population according to different assessment methods

BACKGROUND: The aim of our study was to evaluate a prevalence of early vascular aging (EVA) syndrome and its risk factors, using different methods of arterial stiffness assessment.
METHODS: A random selection of 481 apparently healthy Saint-Petersburg inhabitants aged 25-65 years was performed. Fasting lipids, glucose and blood pressure measurements were performed. We used 3 definitions of EVA syndrome - if vascular age calculated from CAVI exceeded the biological age ≥4 years (EVAvas), if cfPWV-S exceeded 2SD for corresponding age group in Russian population (EVAsphRus), if cfPWV-S exceed 2SD reference value PWV stratified by age (EVAsphRef).
RESULTS: Prevalence of EVA according to EVAvas criteria (82 [17%]) was higher comparing to EVAsphRef (36 [7.5%]) and EVAsphRus (14 [2.9%]), kappa index of criteria agreement was low - 0.08. A higher probability of EVA syndrome, according to EVAsphRef and EVAsphRus, was in subjects with hypertension (OR 2.76 and 11.4), hyperlipidemia (OR=2.5 and OR=3.9), and diabetes type 2 (OR=3.9 and OR=5.9). The probability of EVA according to EVAvas was higher only in case of hypertension (OR=2.6).
CONCLUSIONS: This research emphasizes importance of vascular assessment using different methods of arterial stiffness determination and age consideration, blood pressure level and metabolic profile.
