Unilateral ovarian and fallopian tube agenesis

Unilateral adnexal absence is extremely rare and few cases are reported. It is usually an incidental finding during laparoscopy. Two causes are assumed: prior torsion of the adnexal pedicle or a congenital disorder concerning the genital ridge and the caudal end of the Müllerian duct. However, these remain modest hypothesis since research is still absent. We report a case of a 31-year-old woman admitted to our center for abdominal pain. During diagnostic laparoscopy, besides pelvic inflammatory disease and right polycystic ovary, absent left ovary and tube was noticed. We discuss the etiologic hypothesis and other implications of this condition, such as fertility matters, assuming our case as an adnexal agenesis without concomitant urogenital malformations.
