Urethral sling surgery using a surgeon-tailored polypropylene mesh in patients with stress incontinence

BACKGROUND: This study evaluated the outcome of urethral sling surgery using a surgeon-tailored polypropylene mesh in patients with a urodynamic diagnosis of urinary incontinence.
METHODS: A questionnaire investigated the pre- and post-operative status of 100 randomly selected patients submitted to transobturator sling surgery using a tailored polypropylene mesh. Surgeries were performed between 2009 and 2014 in the Santa Casa de Misericórdia Hospital in Vitória, Espírito Santo, Brazil.
RESULTS: Mean age was 57.2 years. The prevalence of overweight or obesity classes I-III was 67%. The mean number of pregnancies was 4 and the mean number of vaginal deliveries was 3. Regarding ethnicity, 48% of the participants described themselves as white, 45% as brown-skinned and 7% as black. All patients had complained of urinary leakage and, in addition to the principal complaint, 53% had also complained of urge incontinence, 76% of stress incontinence and 66% of nocturia. Urodynamic evaluation had diagnosed stress incontinence in 80/100 cases and mixed incontinence in 20/100 cases. Of the patients with a diagnosis of stress incontinence, 92.5% were satisfied with surgery compared to only 50% of the patients with mixed incontinence (P=0.000). Overall, 84% of the patients evaluated were satisfied with the surgical procedure, with only 16% stating that they were dissatisfied.
CONCLUSIONS: The use of a surgeon-tailored polypropylene mesh in this type of surgery represents a safe, inexpensive option, with few complications and a consequent improvement in patients’ quality of life. The high satisfaction rate following this procedure supports the use of this technique.
