Cervical amputation after application of the Arabin cerclage-pessary in a dichorionic twin pregnancy

Preterm birth is one of the causes of perinatal morbidity and mortality. The traditional treatment for a short cervix has been the cerclage in addition with the progesterone administration. Recently, the idea of using vaginal pessaries as a new less invasive method of treatment for the same indication is under discussion. The use of the Arabin cerclage-pessary in a 20-year-old woman with dichorionic twin pregnancy and an extremely short cervix, is presented. The patient had a spontaneous premature rupture of amniotic membranes at 31 weeks and 3 days of gestation and underwent emergency cesarean section. Following surgery, cervical pessary was removed and there was an amputation of the cervix, most likely due to a wrong positioning of the pessary as it turned out to be put in reverse, with the concavity facing the body of the uterus.
