Bilateral transient osteoporosis of the hip with femoral fracture during pregnancy

Transient osteoporosis (TO) of the hip is a rare and self-limited syndrome, characterized by sudden onset of joint pain, followed by local osteopenia and spontaneous healing. It is a diagnosis of exclusion and its etiology is still unknown - although there is an association with pregnancy. The most severe complication is fracture of the hip, which may be avoided by early diagnosis and management. We present a case of a healthy woman, in the third trimester of gestation, that developed a sudden, severe and progressive pain in the groin. Several weeks later, associated with bad positioning, she suffered a pathological hip fracture. On the radiographs, a diffuse bilateral osteoporosis of the hip was noted, raising the suspicion of TO. The patient was immediately submitted to a percutaneous osteosynthesis, and recommended protected weight bearing; the rest of the pregnancy was uneventful and she delivered a healthy baby at 36 weeks.
