Obstruction of the uterine cervical canal due to benign or malignant causes leads to impaired uterine drainage and accumulation of infected material in uterine cavity, ultimately leading pyoderma. In August 2014 an 82-year-old woman presented to our hospital with fever, nausea, and vomiting for 3 days. The general status of the patient was poor and she had severe pain. An informed consent was obtained from the patient for laparotomy. The patient recovered progressively on intense antibiotherapy and was discharged 15 days after surgery. In conclusion, pyometra is a serious gynecological condition with risk of significant morbidity and mortality or spontaneous uterine perforation, although the latter is rare. Early diagnosis is central to appropriate patient management.
