Sacrococcyeal teratoma (SCT) is the most common congenital neoplasms tumor and it is classified in four types; only a small number of tumors belongs to type IV. We report a case in which US and MRI together correctly made a prenatal diagnosis of a type 4 cystic SCT. Prenatal sonographic findings of cystic SCTs may at times be strikingly similar to those of menyngomieloceles and myelocystoceles. MRI clearly shows the relationship of the mass to sacrum and spinal cord; it delineates the extent of compression on pelvic and abdominal organs and demonstrates the internal structure of SCT better than sonography.
