Risk factors to develop mediastinitis in patients with deep neck abscess

BACKGROUND: It is important to analyze the risk factors associated with mediastinitis in patients with deep neck abscess.
METHODS: This is a cross-sectional, retrospective, observational and analytical study. A review of files from the patients with a diagnosis of deep neck abscess, treated in the period from January 1, 2018, to May 1, 2019, was carried out at the Head and Neck Surgery Department of our Institution.
RESULTS: A total of 113 patients were included, of which 24 (21%) presented mediastinitis, and 26.2% were men. The median age was 53±13 and mortality was 16.6%. There was difference in the patients who presented mediastinitis with respect to the affected area, with greater involvement of the retropharyngeal space and mainly visceral (P<0.05). In the logistic regression model, it was found that the factors related to the presentation of mediastinitis were the history of tracheostomy, as well as the involvement of the retropharyngeal and visceral space.
CONCLUSIONS: Deep neck abscesses continue to be a common pathology in our environment, with mediastinitis being the most frequent and most serious. The aim of the study was to be able to know the factors that influence its development, such as a history of tracheostomy, or the spaces involved.
