Hiatal hernia is an increasingly frequent pathology. The clinical case demonstrates how a voluminous hiatal hernia in an elderly patient with multiple comorbidities can be difficult to manage. Surgical treatment, which is the standard in an emergency regime, is often characterized by postoperative complications. These complications can be treated with a multidisciplinary approach, involving different specialists such as radiologists and endoscopists. The multidisciplinary approach is fundamental in the resolution of complex clinical cases.
