Fatal myonephropathic metabolic syndrome after surgical repair of popliteal aneurysm rupture with distal occlusion

Ruptured popliteal artery aneurysms (PAAs) are exceptional and diagnosis and treatment remain difficult. An 85-year-old man presented with a painful mass in his popliteal fossa and acute limb ischemia. Exams revealed a ruptured PAA with distal occlusion, venous bypass grafting was performed and followed by renal function deterioration, severe metabolic acidosis, and rhabdomyolysis. Reperfusion syndrome led to multiple organ failure and death. Increased awareness of the misleading presentation of ruptured PAAs could limit the delay of surgical repair and controlled reperfusion and hemofiltration could limit the consequences of reperfusion syndrome as this condition remains often fatal.
