We present a case of midgut volvulus and recurrent volvulus after partial Ladd procedure in a 15-year-old boy with a history of diamond shaped duodenoduodenostomy as a newborn. The patient had a markedly elongated mesenteric pedicle without any other features of malrotation. Both, recurrent volvulus after partial Ladd procedure and isolated elongated mesenteric pedicle are extremely rare conditions. Literature review showed eleven published cases of recurrent midgut volvulus after Ladd’s procedure. An isolated elongated and narrow-based mesenteric pedicle without malrotation could only be found in two cases in our literature review. Its combination has not been described yet.
