Chondrosarcoma is a rare neoplasm of the head and neck. It is a tumor that only in the 20% of cases occurs with only intraluminal extension. The most commonly reported symptom are dyspnea, nonproductive cough and wheeze. Computed tomography (CT) is the imaging method of choice for tracheal chondrosarcomas; bronchoscopy represents the gold standard for the diagnosis, as it allows the collection of pathology specimens. In our case the patient had a lesion of 20 mm that involved 2 rings of middle-third trachea. The outer surface of trachea was totally free from any neoplastic invasion. The use of intraoperative bronchoscopy was necessary and useful to provide a sure tracheal resection with free margins from tumor.
