Limited data regarding heart surgery on patients who have been previously pneumonectomized are available. In these patients the anatomical alterations and the preexisting pulmonary functional deficit increase considerably the postoperative morbidity and mortality and postoperative care necessitate deviations from standard. A triple coronary artery bypass grafting was performed in a 74 years old man with history of a left pneumonectomy 26 years ago and recent stroke, presented with unstable angina. His postoperative course was complicated and prolonged but the patient ultimately recovered with good functional status.
