Direct inguinal blunt trauma is an uncommon but possible cause of bowel perforation in patients with groin hernia. In this case report we want to describe bowel perforation in a 67-year-old patient with bilateral groin hernia due to a directly inguinal blunt trauma managed with loop ileostomy with rod. This case reveals the importance of elective hernia repair and the value of clinical and instrumental evaluation of a patient with groin hernia after direct blunt trauma.
