Efficiency of endoscopic use of cyanoacrylate and Lipidol® in the treatment of a high-grade bile leak after blunt liver trauma: a rare case report

A case of a 23-year-old man with a hepatic parenchymal laceration of V and VIII segments and an hemoperitoneum due to a blunt liver trauma is reported. He underwent an emergency liver packing, complicated by major bile leak. An unsuccessfully percutaneous treatment of the bile leakage was tried, therefore an endoscopic retrograde cholangiopancreatography (ERCP) using a mixture of N-butyl cyanoacrylate (Glubran®) and Lipidol was successfully performed. Bile leak remains the main complication after hepatobiliary surgery and liver trauma. Several approaches to treat biliary leaks have been reported in the literature to date: the endoscopic approach remains the main and first choice. In this paper, we describe a novel endoscopic technique using a mixture of N-butyl cyanoacrylate and Lipidol, associated to a polymeric-stenting placement: very few cases like this have been reported in the literature to date.
