Zero bispectral index through sensor on the mandible during aneurysm clipping surgery: a case report

The Bispectral Index (BIS) is a monitoring modality designed and used for monitoring depth of anesthesia. The mandibular position can be availably used as an alternative in neurosurgery because the standard bifrontal application of sensor interferes with the operative field. I report the zero BIS through sensor on the mandible after intravenous injection of pentothal sodium for neuroprotection. She was a 59-year-old woman under aneurysm clipping surgery because of left middle cerebral artery bifurcation aneurysm showing a wide neck. The BIS fell to zero and slowly recovered for 10 minutes. Sensor attached to chin accurately reflected brain activity. She showed dysarthria with cerebral infarction in left striatocapsular area 2 days after surgery, but she was discharged 2 weeks later because of proper treatment.
