Relationship between HLA-DQA1 and duodenal ulcer perforation

BACKGROUND: Duodenal ulcer disease is affected by environmental factors as well as genetic factors. Some of the HLA-DQA1 gene alleles are found high in Helicobacter pylori infection and duodenal ulcer disease. We describe the relationship of duodenal perforation which of the most important complication of duodenal ulcer with HLA-DQA1.
METHODS: This study included 100 cases with 30 controls, 30 duodenal ulcers and 40 duodenal ulcer perforations. Standard tissue biopsies or urease breath tests were used for H. pylori diagnosis in these groups. Blood samples were taken with ethylenediaminetetraacetic acid (EDTA) tubes. Serum samples for pepsinogen and blood samples for HLA-DQA1 were stored at -20° and -80°, respectively.
RESULTS: Polymerization of HLA-DQA1 0101 was significantly higher in the perforation group (P=0.002, P<0.01). Pepsinogen levels of perforation group was significantly higher than both of the ulcer (P=0.001) and normal group (P=0.001). Also, pepsinogen levels of ulcer group were found significantly higher than the normal group (P=0.001).
CONCLUSIONS: Cases with HLA-DQA1*0101 alleles have genetic predisposition to duodenal ulcer perforation.
