Jejuno-ileal diverticulitis: a rare cause of acute abdomen

Jejuno-ileal diverticulitis is extremely rare clinical entity, with a prevalence that is around 2%. Typically occurs in elderly patients and its manifestations can be very variable. Indeed, it ranges from asymptomatic forms of all, the abutment which is incurred occasional all of the post-mortem examinations, to forms in which the main symptom is anemia due to bleeding of one or more diverticula, up to complicated forms that manifest with painful acute abdomen for perforation of diverticula. In this report we describe a case of a patient of 86 years who presented to the emergency room of our hospital complaining of diffuse abdominal pain for several days. With blood chemistry and instrumental tests to which the patient underwent was brought out a picture of acute abdomen due to a likely colonic diverticular perforation. At the time of exploratory surgery in which the patient has undergone, to the surprise, it has instead found diverticular disease affecting the small intestine instead. The patient was then submitted to resection of the intestinal segment involved by the disease with immediate restoration of intestinal continuity. The subsequent post-operative course has regularly played with the resignation of the patient to standard time at home. We conclude our report with a review of the available scientific literature about this disorder, with the intention to always keep in view this condition as a rare but very dangerous due to acute abdomen.
