Myofibroblastoma is a rare, benign mesenchymal tumour of the breast. Since its first description by Wargotz in 1987, less than 70 cases have been reported in literature, mostly in males. We report a case of a 64-year-old woman with a 2,5 cm lump in the upper-outer quadrant of her left breast. She underwent Ultrasound-guided Fine Needle Aspiration Citology (US-FNAC) of the lesion that showed a mesenchimal proliferation without atypia. Excisional biopsy was performed and histological examination showed a spindle cell proliferation without cellular atypia and aboundant dense fibrous collagen, in absence of mammary ducts and lobules. Spindle cells resulted positive for vimentin, CD34, desmin, H-caldesmon, CD99, bcl2 and actin and were negative for cytokeratins and S-100. The morphologic features, in conjunction with the immunophenotype, were consistent with a myofibroblastoma of the breast (classic type). The patient did not perform any other medical adjuvant treatment in relation to the benign behaviour of these kind of lesions reported in literature.
