A surprise diagnosis in a dentigerous cyst: adenomatoid odontogenic tumor

Adenomatoid odontogenic tumor (AOT) is an uncommon benign tumor of odontogenic origin with a variety of histoarchitectural patterns. The lesions can be solid or cystic and are common in females. They are common in the anterior maxilla and have been reported to occur with many types of odontogenic cysts, however extensive lesions in the maxillary antrum and the association of these tumors with a dentigerous cyst has rarely been reported. A 19-year-old female patient presented with a progressive swelling on left side of the face which on CT scan revealed a large radiolucent lesion associated with the crown of an unerupted canine located in the maxillary antrum. It was provisionally diagnosed as a dentigerous cyst and was enucleated. However the post operative histopathological diagnosis came out to be that of an Adenomatoid odontogenic tumor. A case of a large AOT in association with dentigerous cyst is described along with review of literature. We have made an attempt to highlight the lacuna in our present knowledge about this distinct clinical condition and have tried to fill this up.
