Subfascial aneurysm of the incompetent popliteal perforator vein: case series overview and novel minimally invasive approach presentation

Nowadays the problem of perforator vein aneurysms (PVA) is quite intriguing and neglected. Our knowledge of the incidence and clinical role of this issue is based on very limited data. Aneurysmatic transformation typically occurs with leg and middle-thigh perforators of the great saphenous vein (GSV), whereas there is only a single mention of popliteal perforator vein (PPV) aneurysm. PV is defined as a non-saphenous vein that runs subcutaneously along the posterior fossa and upper-third of the posterior calf, pierces the deep fascia and empties directly into the popliteal vein, usually just laterally and superiorly to the sapheno-popliteal junction (SPJ). PPV is a quite uncommon source of the lower limb varices; however, its prevalence is rather variable - from 4.4% up to 19%6 of individuals. In the case of its incompetence, varicose tributaries could reach a considerable size (up to 15-20 mm in diameter) even without pathological transformation of GSV or small saphenous vein (SSV). We would like to present case series of incompetent PPV with the formation of subfascial juxta-junctional aneurysm and a particular case report with the introduction of novel surgical approach.
