Peripheral venous aneurysm is a rare disease entity and yet little is known about it. We would like to share our experience in treatment of primary external iliac vein aneurysm. A 65-year-old male patient presented with left lower limb swelling. Duplex ultrasound revealed thrombi in the left iliac external vein, common femoral vein and proximal great saphenous vein. A CT-angiogram showed thrombosed external iliac vein aneurysm. Patient was successfully treated conservatively by anti-coagulant and inferior vena cava filter. The recent literature suggests surgical treatment for non-thrombosed venous aneurysm to decrease the risk of complications. On the other hand, there is no sufficient data in literature about treatment of thrombosed venous aneurysm. Our case suggests that conservative treatment of thrombosed venous aneurysm is safe and justified.
