Dermoid cysts are common teratomatous lesions, which infrequently arise in the head and neck area. Very rarely, they appear in the floor of the mouth. An extremely rare case of a lateral dermoid cyst located in the submandibular region is described. Magnetic resonance imaging demonstrated a cystic lesion containing multiple uniformly rounded foci, creating a ‘‘sack-of-marbles’’ sign. The mass was surgically excised via transcervical approach. Histopathologic examination revealed the presence of a dermoid cyst. The pathogenesis of dermoid cysts in this region is discussed, as well as diagnosis and treatment of dermoid cysts of the head and neck are reviewed in light of recent literature.
