The expression of receptor activator of nuclear factor κβ ligand and osteoprotegerin of cholesteatoma in Padang, West Sumatra, Indonesia

BACKGROUND: Chronic suppurative otitis media (CSOM) is a chronic middle ear infection with profuse discharge from the ear through the perforated tympanic membrane. CSOM is classified into two types: with cholesteatoma and without cholesteatoma. Cholesteatoma is a temporal bone lesion coated by a stratified squamous epithelium containing keratin desquamation that can cause bone destruction in the middle ear. One of the cytokines that plays a role in the process of bone resorption is receptor activator of nuclear factor κβ ligand (RANKL) and osteoprotegerin (OPG). The purpose of this study is to determine the expression of RANKL and OPG of cholesteatoma in CSOM patients compared to the expression of RANKL and OPG in normal ear canal skin.
METHODS: A cross-sectional comparative study on 16 samples of cholesteatoma and normal ear canal skin. All samples were examined using real-time polymerase chain reaction (RT-PCR). Data were analyzed with SPSS v. 23 using the Mann-Whitney test.
RESULTS: Cholesteatoma expressed RANKL (19.83±34.36), OPG (1037.60±1710.38) and RANKL/OPG ratio (0.09±0.24). Normal ear canal skin expressed RANKL (0.02±0.06), OPG (5.11±7.15) and RANKL/OPG ratio (0.005±0.02). Statistically, the increased expression of RANKL, OPG and RANKL/OPG ratio is significant (P<0.05).
CONCLUSIONS: There were statistically significant increases in RANKL, OPG and RANKL/OPG expression in cholesteatoma of CSOM compared to normal ear skin.
