Gastro-pleural fistula is an extremely rare and insidious complication of perforated peptic ulcer whose timely diagnosis is extremely important. We present the case of a puerpera that has come to our observation for a hypertensive hydro-pneumothorax caused by a gastro-pleural fistula, whose drainage of the gastric content in the pleural cavity has determined the formation of a voluminous empyema. The imaging methods and the microbiological analysis of the drained pleural fluid have proved to be of fundamental importance in suspecting diagnostics and identifying complications.
