We present a chronic exogenous lipoid pneumonia case and its imaging features. Lipoid pneumonia is a rare disease characterized by intra-alveolar and interstitial fat material accumulation. The pathogenesis of the exogenous form is related to inhalation and aspiration, occasional or chronic, of oil-based substances; the endogenous form can be a manifestation of a miscellaneous lung diseases. Some imaging features are particularly important (above all region-of-interest measurement and Hounsfield Unit density analysis). Correlation with clinical-anamnestic features is mandatory. Sometimes histopathologic confirmation is required.
