Round ligament varicosities (RLV) may present as a palpable soft tissue mass in the inguinal region and should be considered in the differential diagnosis of groin swelling, especially during pregnancy, as RLV clinically often mimic an inguinal hernia. We report a case of a 36-year-old woman, presented at 30 weeks’ gestation with painful swelling in the left groin, clinically suggestive for an inguinal hernia. The color Doppler ultrasonography, performed also during Valsalva maneuver, in supine and upright position, revealed the correct diagnosis.
