We report the case of a 53-year-old man suffering from worsening dysphagia for several months and initial anemia. A CT scan with contrast medium was performed which demonstrated the presence of a voluminous expansive lesion of the distal third esophagus. Histological examination defined the diagnosis of esophageal gastrointestinal stromal tumor (GIST). The aim of this article is to illustrate the CT aspect and the metabolic behavior of the esophageal GIST, a rare localization, and to correlate the imaging with the anatomical and pathological features. Then expose the main differential diagnoses with the other esophageal neoplastic lesions.
