Multifocal pancreatic tumors are rare (about 5%) and frequently associated with hereditary conditions, intraductal papillary mucinous neoplasm, pancreatic intraepithelial neoplasia and chronic pancreatitis. We describe a case of two synchronous invasive ductal pancreatic adenocarcinomas, located in the head and in the tail of the gland, detected by multidetector computed tomography. Total pancreaticoduodenectomy and splenectomy were performed. At histopathological examination the pancreatic tissue between the two lesions was normal, without inflammatory or preneoplastic changes.
