Embolization of a pulmonary sequestration prior to video-assisted thoracoscopic surgery: the efficacy of POD system

Intralobar pulmonary sequestration is a rare pulmonary malformation that consists in the presence of nonfunctional lung tissue, not connected with the tracheobronchial tree and with an aberrant systemic supply. Sequestrations are classified into extralobar and intralobar forms, with the former being defined as a lung segment contained by its own pleural investment while the latter as a part of lung surrounded by normal pulmonary tissue without its own pleural covering. Few patients are asymptomatic, while the majority of them presents recurrent episodes of respiratory infection and, less frequently, hemoptysis. Diagnosis is based mainly on the imaging finding of the abnormal arterial supply, which most commonly arises from the descending thoracic aorta; the traditional treatment consists in surgical resection. We present a case of an asymptomatic adult with an incidental diagnosis of intralobar pulmonary sequestration with a single feeding artery arising from the celiac trunk, treated successfully with VATS resection after trans-arterial embolization with Penumbra Occlusion Device (POD, Penumbra Inc, Alameda, CA, USA), a new arterial occlusion device.
