Aorto-caval fistula is a rare complication affecting less than 3-6% of cases of abdominal aortic aneurysm rupture; the simultaneous presence of pulmonary embolism is an even less frequent occurrence. The clinical manifestations are not unique, and imaging is therefore the fundamental element to allow an early diagnosis and ensure an adequate treatment. We report the case of a 66-year-old man with aorto-caval fistula complicated by pulmonary embolism. It was diagnosed by angio-CT and treated with an endovascular approach by the positioning of an aortic endoprosthesis.
