Hepatocellular carcinoma (HCC) has different growth patterns and, among these, exophytic pattern is quite rare (less than 3%). However, lack of awareness of this possibility can lead to severe diagnostical mistakes especially about the origin of the lesion itself. We discuss a case of exophytic HCC with extension into the adrenal space and we review the literature to familiarize with the possible differential diagnosis and the prognostic and therapeutic consequences of a correct diagnosis.
