Renal lymphoma patterns and differential diagnosis

The organ of the genitourinary system most affected by extranodal dissemination of lymphoma is the kidney. Non-Hodgki’s lymphoma, especially the diffuse large B cell lymphoma, is the type that most leads to renal involvement. Different mechanisms of proliferation determine the formation of different imaging patterns: multiple masses, renal invasion from retroperitoneal mass, diffuse renal involvement, solitary lesion and perinephric disease. However, it is possible to detect a mix of these patterns in the same patient. In this paper we describe the imaging characteristics of renal lymphoma in various techniques: ultrasound, computed tomography and magnetic resonance imaging. Computed tomography is the technique of choice for the study of patients with suspected renal lymphoma, allowing the detection of lymphoma lesions and the evaluation of the extension of the disease to the surrounding anatomical structures. Magnetic resonance imaging is crucial for the detection of lymphoma renal lesions, for the local extension of the disease and is an important aid in patients with renal insufficiency and/or allergy to the non-ionic organo-iodate contrast medium. Ultrasound can also be used as first diagnostic test in this type of patients. We also describe the differential diagnosis of renal lymphoma, highlighting how each single pattern of disease is part of differential diagnosis with varoius renal diseases.
