The unilateral pulmonary artery agenesis (UPAA) is a rare congenital anomaly characterized by lack of development of one of the two main branches of the pulmonary artery; may be isolated or associated with other cardiovascular pathologies. The diagnosis is insidious due to the absence of typical clinical signs and the rarity of this affliction that is sometimes completely asymptomatic. We describe the case of an emergency department patient with suspected pulmonary thromboembolism in which CT with contrast agent, “gold standard” imaging technique in the study of chest vascular anatomy, documented a UPAA until then unknown.
