Correlation between tumor tissue heterogeneity evaluated through Texture Analysis and plasmatic levels of Ca125, HE4 and vitamin D in patients with ovarian cancer

BACKGROUND: To evaluate the correlation between texture parameters and humoral biomarkers levels in patients with ovarian cancer.
METHODS: Twenty-five patients with ovarian carcinoma underwent a contrast-enhanced multi-detector CT for disease staging. The same day of the CT scan, a blood sample was obtained from each patient for the evaluation of the following humoral biomarkers: Ca125, HE4 and vitamin D. Using a commercial software for texture analysis, a ROI including the solid portion of the tumor were delineated on CT images. The following texture parameters were analyzed within the ROI: mean pixels (M), mean positive pixels (MPP), entropy (E), skewness (S) and kurtosis (K). Biomarkers and texture parameters were correlated using the Spearmnan correlation test (P<0.05) and the linear regression test.
RESULTS: The mean blood concentration of plasmatic biomarkers resulted as follow: Ca125 993.4 U/mL, HE4 546.8 U/mL e Vitamin D 13.26 ng/mL. Vitamin D showed a significant negative correlation (P=0.01) with E, resulting lower in tumors presenting an higher E value. Vitamin D showed also a significant negative correlation with MPP (P=0.001), M (P=0.01) and S (P=0.01) values. Moreover, a significant linear regression (<0.001) was found correlating Vitamin D with E and MPP values. Between the texture parameters and the blood concentration of Ca125 and HE4 any significant linear regression was found.
CONCLUSIONS: Vitamin D blood concentration in patients with ovarian cancer resulted to be lower as higher was the values of some texture parameters expressing greater tumor heterogeneity.
