Otoneurological and multidisciplinary strategy to evaluate the evolution of vascular statoacoustic impairment. A 10-year follow-up

Background and aims. The authors evaluated the evolution of vertebrobasilar insufficiency in patients using an otoneurological and multidisciplinary strategy consisting of a multi-specialist clinical examination and instrumental tests, like Doppler flowmetry of the epiaortic vessels and image diagnosis, as well as evaluation of the audiovestibular balance. Patients were monitored during a 10-year follow-up.
Methods. The study included a total of 172 patients suffering from hearing loss and vertigo probably caused by vertebrobasilar insufficiency. All patients underwent the aforesaid diagnostic protocol. Tests were repeated after ten years in 166 patients, given that four patients had died from vascular causes and two from cancer.
Results. The results obtained show the progression of deafness predominantly affecting the acute frequencies, whereas in the case of the vestibular component a progressive prevalence of central damage was noted. The data obtained also underlined the importance of Doppler flowmetry and even in particular the search for nystagmus caused by vertebrobasilar deficit in the diagnosis and monitoring of this disease.
Conclusions. The authors conclude by emphasising the importance of a fully multidisciplinary and instrumental approach in order to make a correct evaluation of patients with statoacoustic damage. With regard to the incidence of cerebrovascular attacks in these subjects, there is no evidence at present to affirm that statoacoustic vascular pathology may be regarded as a premonition of this event.
