The case of a 61-year-old woman who at admission presented a left pleural transudate effusion is reported. A multidetector CT scan with angiographic acquisition permitted a diagnosis of thrombosis of the upper vena cava astride the outlet of the azygos vein, thus classifying the pathology within the context of a “mediastinic syndrome”.
