Bronchoscopic diagnostic approach to mediastinal diseases has been revolutionized in the last 2 decades by the widespread use of transbronchial needle aspiration (TBNA). This is a low cost, minimally invasive procedure with a low complication rate. The technique is a versatile one, as it can be used to sample mediastinal, submucosal, peribronchial, and peripheral pulmonary lesions. In the specific setting of the mediastinal diseases, TBNA has been successfully used in diagnosis of neoplastic, inflammatory, infectious, and developmental disorders involving the middle mediastinum.
